By checking this box you acknowledge and agree that membership cancellations must be submitted at least five (5) calendar days prior to the first day of the following month in order for the cancellation to be effective for the current billing cycle. Any cancellation request received fewer than five (5) days before the first of the month will be processed for the subsequent billing cycle, and the member will be responsible for any applicable dues for that month. By agreeing the member confirms understanding and acceptance of this cancellation policy.
